* I acknowledge that this athletic event is a test of a person's physical and mental limits and carries the potential for death, serious injury and property loss. The risks include, but are not limited to: actions of other people, weather/other natural conditions and other water activities not conducted by Columbia Basin SUP.

* I accept the equipment, as is, for my use and rental. I accept that there are no warranties applicable to the rental or lease of the equipment and all warranties expressed or implied are excluded. I accept that I will personally examine the equipment prior to my use and report any issues or flaws that I may find unsafe for use. I accept full financial responsibility for the care of the equipment while in my possession. I accept that I will be held responsible for the full retail value of any equipment that is lost, stolen, damaged or otherwise not returned in the same condition as it was originally rented to me.

* I agree to return all rental equipment by the agreed time and date or I will incur a late fee.

* I agree to wear a personal flotation device and leash at all times.

* I agree that I will stay in boundaries as defined by Columbia Basin SUP. Failure to do so will result in revocation of privileges and a rescue fee.

* I certify that I am physically fit, have sufficiently trained for participation in this event and have not been advised otherwise by a qualified medical professional. I agree to take full responsibility for any injuries, claims, costs and judgments that I may inflict upon others. I will refrain from acting in a manner which may cause injury to myself or others. I agree that I am responsible for the loss or damage of my personal belongings.

In consideration of my application and permitting me to participate in this event, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors , and assigns as follows: (A) Waive, release, and discharge from any and all liability for my death, disability, personal injury, property damage, property theft or actions of any kind which may hereafter accrue to me or my traveling to and from this event, the following entities: Columbia Basin SUP, LLC and Black Widow Boards, and their directors, officers, employees, volunteers, representatives and agents, the event sponsors and event volunteers, (B) indemnify and hold harmless all entities or persons mentioned in this paragraph from any and all liabilities or claims made by other individuals or entities as a result of my actions during this event. I also hereby consent to any commercial uses of my image and/or identity by Columbia Basin SUP, without compensation or payment to me, and waive my rights under the Washington Right of Publicity Act.

* I hereby certify that I am at least (18) years of age; am able to operate this equipment and understand the equipment's use and function; that I know how to swim; and that I am not under the influence of alcohol or drugs. I have read this release of liability, waiver of legal rights and assumption of risk. I fully understand it's contents. I sign this waiver freely and voluntarily without any inducement and agree to be bound by it.
